Hospices in Surrey have joined with Surrey County Council, Public Health, the NHS, charities and faith groups. We are writing a Joint Strategic Needs Assessment for End of Life and for Bereavement. This will help shape the development of services in the future. You can use the form on this page to submit your experiences to help us write the JSNA.
We’re looking at how we can ensure our services best support families and loved ones who face the end of life of a baby, young child or adult. If you have needed our services or have been bereaved or experienced loss, we’d like to hear from you. All your information will be kept confidential and will not be shared.
