Anna, our social worker for transition, will explain the changes in law from children to adult care, the Mental Capacity Act, Continuing Care and Social Care. Most importantly, we will tell you about the resources and services available to support you.
Please come along to listen or ask questions. We will follow up with 1:1 meetings as needed, especially if we can’t answer all of your questions during the session.
The event will be held via Teams. This will be sent to you via text before the event.
