Have your say on end-of-life care in Surrey

Hospices in Surrey have joined with Surrey County Council, Public Health, the NHS, charities and faith groups. We are writing a Joint Strategic Needs Assessment for End of Life and for Bereavement. This will help shape the development of services in the future. You can use the form on this page to submit your experiences to help us write the JSNA.

We want to hear about your experiences of services for babies, children, young people and adults who have reached the end of life/ time of passing. We also want to hear from families and loved ones who have been bereaved, or experienced loss. All your information will be kept confidential and will not be shared.
